Microneedling protocol: a road map

At the Skin Care Center, we’ve perfected a unique protocol for Percutaneous Collagen Induction (PCI) skin rejuvenation that ensures exceptional results. Our process is meticulously designed to prioritize safety and efficacy:

1. Preparation: We start by cleansing your skin with a gentle antiseptic soap, ensuring that all makeup and sunscreen creams are completely removed. Our certified staff complies with OSHA blood-borne pathogen certification criteria, using protective goggles, sterile gloves, and masks.
2. Topical Anesthesia: To ensure your comfort, we apply a topical Lidocaine and Prilocaine cream (2.5% lidocaine; 2.5% prilocaine) with saran wrap. This anesthesia is applied for 10 to 15 minutes to one area at a time, especially in sensitive areas like the forehead and perioral and periorbital areas.
3. Cleansing: We remove the Prilocaine cream with sterile gauzes, cleanse your skin with HIBICLINS (chlorhexidine gluconate 4% w/v), and perform a final wash with sterile saline.
4. Customized Microneedle Depths: Our experienced staff selects the appropriate microneedle depths based on the specific treatment area. Depths range from 0.25 mm to 1.0 mm for areas like the forehead, face, eyelids, lips, chin, neck, and decolletage. Depths of 1.0 mm are chosen for upper extremities, mild-moderate wrinkles, lax crepey skin, hypo/hyperpigmentations, and hypertrophic scars.
5. Microneedling: Using sterile gloves, we perform gentle, steady gliding passes with hyaluronic acid as a wetting agent. Each section of the face and neck undergoes at least 10 to 20 passes in vertical, horizontal, and oblique directions.
6. Specialized Techniques: For severe wrinkle lines, deeper pigmentations, and deep acne scars, we employ a rocking stamp method to ensure comprehensive treatment.
7. Observation: We cleanse the treatment sites with sterile saline and closely observe the endpoint, looking for uniform pinkness or punctate bleeding. Any uneven or missed areas receive additional passes.
8. Cosmeceutical Massage: We massage cosmeceuticals containing cytokine growth factors into the treated skin within 5 to 15 minutes, following the same pattern and frequency as during microneedling.
9. Moisturization: Hyaluronic acid moisturizer is applied to keep your skin hydrated and supple.
10. Post-Treatment Care: After two days of twice-daily applications of cytokine growth factors, we introduce a regular skincare regimen. This includes sunscreen, pigment inhibitors, antioxidants, vitamins A and C, and mineral-based makeup.

In our Surgical Center, the protocol aligns closely with that of the Skin Care Center, with a few enhancements. Surgical procedures are considered when employing PRP application with plasma-poor plasma (PPP) used as the wetting agent. After microneedling, PRP is massaged into the treated skin within 3 to 5 minutes, following the same pattern and frequency passes as performed during microneedling. Additionally, microneedling depths between 0.5 to 2.5 mm are recommended for prominent scars, deep rhytids, and hair-stimulation.
